Accurate information so we can reach you — your full name, a valid email address, and a postal address.
Good-faith & accuracy statements
A statement that you believe in good faith the use is not authorized, and that the information you’ve provided is accurate — under penalty of perjury.
Your signature
A physical or electronic signature of the copyright owner or their authorized representative completes the notice.

You can file a counter-notice
If you believe material of yours was taken down in error or misidentified, email our copyright agent with a written counter-notice. Include your contact details, identify the removed material and its previous location, and add a statement under penalty of perjury that the removal was a mistake. We'll review it and respond promptly.
